Dog license forms can be downloaded and should be mailed to the City Clerk's office or returned in person. Mail-In dog registration form for each dog to be registered send: 1) The completed form 2) Original current rabies certificate (unless current rabies certificate is on file from prior registration) 3) Original spaying/neutering certificate (unless dog has been previously registered as spayed or neutered) 4) A check or money order made payable to the City of Revere Dog license request form: https://cdn.branchcms.com/GB7r14nbKy-1182/docs/2022DogLicenseApplicationEnglish-2.pdf Annual License Fee: Neutered or Spayed dog: $10.00 Unneutered or Unspayed dog: $25.00 Late Fines: Dogs Licensed after June 30th, 2022 $15.00.
