Requests for a certified copy of a death certificate can be obtained online at https://epay.cityhallsystems.com/ or in person or mail. For requests in person please bring the name of each party, the date of the marriage, and $10 cash, check, or money order. For requests by mail, please fill out this request form from https://cdn.branchcms.com/GB7r14nbKy-1182/docs/Death-Certificate-Request-Form.pdf send it to Office of the City Clerk Revere City Hall 281 Broadway Revere, MA 02151 with a check or money order. DO NOT mail in cash. Checks and money order can be made payable to the "City of Revere"
