How do I apply for an accessible parking spot on my street?

Send the following documents preferebly by email to disabilites@revere.org. But if that is not possible application can be sent by mail to: Revere City Hall Attn: Commission on Disabilities 281 Broadway Revere, MA 02151. For mail-in application please ensure you have a copy for yourself and do not send any original documentation. Please fill out a Driver Application form or Passenger Only Application form, Medical Documentation Application form which needs to be filled out and signed by a medical professional. Documents A Also Needed: Please send in a copy of the following along with your applications. Car registration at your Revere address, Disabled Parking Placard showing your photo, ID number, and expiration date, your Massachusetts drivers license, The doctors prescription for your mobility device, if you have one. Examples of mobility devices include canes and wheelchairs. If you drive your car, even if its just some of the time: print out and complete the driver application - check the box at the top of the application that says 'New'. If you are only a passenger in the car and do not drive: print out and complete the passenger application.
